Champlain Community Care Access Centre


The Champlain Community Care Access Centre (CCAC) is dedicated to supporting and enhancing the quality of life, independence, health and well-being of individuals in the community by offering a single point of access for community services. The CCAC is one of 14 Community Care Access Centres in Ontario, and serves over 55,000 clients every year in the eastern portion of the province, including Cornwall.

The CCAC is funded by the Ministry of Health and Long-Term Care. This means that CCAC services are covered by OHIP.

The CCAC is staffed by professionals who will assess your needs, determine your requirements for care, answer your questions and develop a customized care plan that meets your individual needs.
